Acceptable Forms of Identification

One (1) Primary or Two (2) Secondary IDs are required:


Primary Photo ID

Primary Photo ID is required to establish proof of the identity of the person requesting a protected vital record. Acceptable forms of Primary Identification are:

Must Provide One (1)

  1. Current state-issued driver’s license (address must match requestor’s address on application)
  2. Current state-issued non-driver photo ID card (address must match requestor’s address on application)
  3. Current Passport or Visa (must include photo)
  4. Current U.S. military ID
  5. Current Department of Corrections photo ID card dated within the last year
  6. Current state or U.S. government agency photo ID card (for persons requesting certificates as part of that agency’s business)
  7. Current student ID card with copy of transcript

Secondary Identification

If a Primary Identification is not available, two Secondary Identification items of different types may be used to establish proof of identity. Acceptable forms of Secondary Identification are:

Must Provide Two (2)

  1. Temporary driver’s license
  2. Current utility bill with current address
  3. Car registration or title with current address
  4. Bank statement with current address
  5. Pay stub with current address
  6. Income tax return/W-2 form showing current address
  7. Letter from government agency dated within the last six months and showing current address
  8. State-issued concealed weapon permit showing current address
